Forms

The following forms are used in the disability accommodation process. Each form is available as a downloadable document.

ADA Test Intake Form

Required when a student will test with Student Disability Services. Complete this form fully and submit it along with the test to SDS by 2 p.m. the business day before the scheduled test time. View the ADA Test Intake Form.

Student Declining Accommodations Form

Used when a student chooses to decline testing accommodations for a single test, quiz, or the semester. Complete this form with the student and keep the signed copy for your records. View the Student Declining Accommodations Form.

Syllabus Statement

Faculty are asked to include a statement about disability accommodations in their course syllabus. Below is a sample statement provided by Student Disability Services for your reference. You are welcome to use this statement, one provided by your department, or one created your own.

If a student contacts you about having a disability or needing accommodations, share Student Disability Services contact information with the student and refer them to SDS to begin the registration process.

Sample ADA Syllabus Statement

In compliance with the Americans with Disabilities Act (ADA), UAFS seeks to provide reasonable accommodations and services to students who have documented disabilities. Students who believe they are eligible to declare a disability for the purpose of requesting and receiving accommodations must register with Student Disability Services in Flanders, Room 115. Students will be required to submit documentation of their disability.
