If you have a concern or something you would like us to review, you can share it here.

Use this form to report a concern or provide feed back about a service, process, or interaction. Your input helps the university review concerns and improve the student experience.

When to use the Student Complaint Form

This form is intended for general concerns and feedback. 

Some concerns follow a specific university process and should be reported through the appropriate office:

  • Discrimination based on a protected class
  • Sexual harassment, sexual assault, or other sex-based concerns
  • Disability accommodations
  • Grade appeals or student conduct appeals

If your concern falls into one of these areas, use the appropriate process so it can be reviewed by the correct office. 

If you're not sure where your concern belongs, you can still submit this form. The university will review your submission and route it when appropriate.


What to Expect

After you submit the form:

  • Your concern will be reviewed by the appropriate office
  • You may be contacted if additional information is needed
  • Some concerns may be handled through a specific process, such as an academic or conduct review.
